ELIGIBILITY
* If you have received any vaccine in the 14 days prior to your COVID vaccine appointment, you will not be eligible to receive your COVID vaccine. You must wait at least 14 days in between vaccines.
* Please do not sign up for a first dose vaccine if you recently tested positive for COVID-19. The CDC recommends that as reinfection is uncommon in the 90 days after initial infection, a vaccination should be deferred for at least 90 days from the date of a positive test.
To be eligible for this clinic you must be in one of the Governor's Priority Categories and be a Florida resident.+
- Long-term care facility residents and staff
- Persons 65 years of age and older
- Health care personnel with direct patient contact
- K-12 school employees 50 years of age and older
- Sworn law enforcement officers 50 years of age and older
- Firefighters 50 years of age and older
- Others that have been determined by a physician to be extremely vulnerable to COVID -19
Extremely Vulnerable
According to the Governor’s Executive Order 21-47 persons deemed to be extremely vulnerable to COVID-19 may receive a vaccination with a physician’s certification. A COVID-19 Determination of Extreme Vulnerability form signed by your physician must be brought to the vaccination event.
+ On January 21, 2021, State Surgeon General Dr. Scott Rivkees signed a Public Health Advisory prioritizing Florida residents for COVID-19 vaccinations given in Florida. Prior to the first dose of COVID-19 vaccine, every recipient must provide a Florida Driver’s License, State ID or two other forms of documentation, demonstrating Florida residency consistent with the criteria set forth in 381.986(5)(b), Fla. State.; or proof recipient is present in Florida for the purpose of providing health care services involving direct contact with patients. COVID-19 Vaccine Screening and Consent Form TO REGISTER
Check your entries for accuracy!
- Select your preferred time slot. Make sure to scroll down to view all of your options.
- From the drop-down box, select 1 or 2 tickets. If you select 2 – be certain that each ticket has a different name. Duplicates will be deleted. Each person must have a ticket to obtain a vaccine.
- For each ticket, enter the name of the person as it appears on their ID. Make sure that you have entered different names on each ticket and when complete, press Register.
- Then select View Tickets, and click on the order.
- Make sure all information is accurate. If it is not accurate, select edit and make necessary changes.
- When finished, select Print Tickets. This will create a document that will need to be saved and then sent to a printer. If you cannot print, you can pull up Eventbrite on your smartphone and show your ticket.
- Download and complete the COVID-19 Vaccine Screening and Consent Form to bring with you to the appointment. If you are not able to print the consent form, you will be provided a blank one to complete on site.
If you sign up for multiple slots, please be certain to delete the ones you do not intend to use so that others can be vaccinated. www.eventbrite.com
How do I change the name on my ticket?
- Log into your Eventbrite account. Then go to Tickets. The order will always be associated with the email address you entered during registration. If you made a typo, changing the email address on your ticket will not make it show in the Eventbrite App. In this situation, you’ll need to use the order confirmation sent to your email address.
- Click the name of the event to select your order.
- Click "Edit".
- Click “Edit” on the individual ticket and change the name.
How do I cancel my ticket?
Please cancel your ticket as soon as possible and not later than 24 hours prior to the event so that others can take advantage of the opening.
- Go to "Tickets," select your order and choose "Cancel Order".
- Go to your order. Log in to your Eventbrite account, then click your order on the Tickets page.
- Choose "Cancel Order".
- Select "Cancel Order" (next to your order details). Then choose “Yes, cancel this order".
- Check for an email from Eventbrite. The attendee and event organizer receive an email to confirm cancellation.

DAY OF YOUR APPOINTMENT
If you do not feel well, please do not come for your appointment and contact your doctor. COVID infection symptoms are many and we do not wish to have anyone exposed at a clinic. We will be happy to reschedule you for a future clinic, just send an email to Info.SLCEvents@FlHealth.gov to let us know you wish to get a future invitation
1. Arrive at your scheduled appointment time. Arriving early will not gain you access prior to your appointment time.
2. Bring:
- Completed consent form
- Registration ticket(s)
- Valid ID(s)
- Verification of priority status
If needed, please also bring:
- Proof of seasonal residency (see links at the bottom of this message).
- A COVID-19 Determination of Extreme Vulnerability form signed by your physician.
- Proof of healthcare provider role, if appropriate.
3. This event requires walking and standing in lines to get to the vaccination point. Lines may be long, please plan accordingly.
4. A 15 to 30-minute observation period is recommended after your vaccine is administered.
5. You must wear a mask and maintain physical distance of at least 6 feet at all times.
Accepted forms of identification:
- To verify age and address include Driver's License, passport or other government issued picture ID with address and date of birth.
- To verify Priority Status include employer ID badge, recent paystub, W-2, tax form.
- To verify healthcare provider role with current direct patient contact include state medical license/certificate, employment badge/ID, recent pay stub for employer, letter from place of employment about your role on letterhead, W-2, tax form, or related documents.

- You must enter a valid email to receive notifications.
- Be certain to check for accuracy before submitting to ensure you will get notices about your appointment.
- If you currently do not have an email address, you can create one by following the directions below.
To sign up for Gmail, create a Google Account. You can use the username and password to sign into Gmail and other Google products like YouTube, Google Play, and Google Drive.

- Follow the steps on the screen to set up your account.
- Use the account you created to sign into Gmail.
